CHRONIC DISEASE MANAGEMENT
We provide proactive and comprehensive management for patients with chronic diseases, including:

Identifying patients with chronic conditions (hypertension, diabetes, congestive heart failure, COPD, etc.)

Shifting chronic disease management from reactive, episodic care to a planned, proactive approach
-
Incorporating input and treatment from allied healthcare professionals (specialists, nurses, dietitian, physiotherapist, occupational therapist, social worker, etc)
URGENT CARE
We provide medical care to patients with non-life threatening minor acute illnesses including:

sinus, throat & ear infection

urinary tract infection

musculoskeletal pain

migraine
-
mild rash
-
minor cuts and burns
